Memorial Hermann Brings on PCMHs, an ACO in the Works

The physician network of Memorial Hermann Healthcare System in Houston is in the process of transforming several medical practices into patient-centered medical homes.

According to a news release, six practices in the Houston-area are working with TransforMED, a non-profit subsidiary of the American Academy of Family Physicians that offers consultation to physicians looking to start PCMHs.

The Houston practices are seeking to earn Level 3 PCMH accreditation with the National Committee on Quality Assurance. While six practices are participating in the pilot program, Memorial Hermann expects to have as many as 220 primary care practices in the network on board and certified as PCMHs within the next two years.

Memorial Hermann's physician network — MHMD — is also working alongside TransforMED to prepare for participation in a "soon-to-be-established" accountable care organization of the Memorial Hermann Healthcare System.
