Manhattan's Physician Group in New York City has opened a multi-specialty patient-centered medical home in Harlem to provide coordinated healthcare to patients, according to a news release.
Health plan and management company EmblemHealth will provide "point-of-care" support to patients in the Harlem facility to help with medication and treatment adherence, home care and community resources, according to the release.
The physician group of 70 board-certified providers recently received Level 3 Patient-Centered Medical Home recognition from the National Committee for Quality Assurance.
