Aetna, Houston-based Memorial Hermann Health System and the MHMD Memorial Hermann Physician Network have formed an accountable care organization, effective April 1.
The ACO aims to offer a more coordinated and personalized experience for patients, cost savings for employees who use physicians in the network and better health outcomes.
Physicians in the ACO will be rewarded for meeting the following quality, efficiency and patient satisfaction measures:
• Increase in percentage of members who receive recommended preventive care and screenings
• Better management of chronic conditions
• Reduced avoidable readmissions
• Reduced emergency room visits
