A dozen practices in the Haddon Heights, N.J.-based multispecialty physicians network Lourdes Medical Associates — part of the Camden, N.J.-based Lourdes Health System — have received patient-centered medical home recognition from the National Committee for Quality Assurance, according to a Philadelphia Inquirer report.
The practices are now included in NCQA's Physician Practice Connections PCMH directory, which means the medical teams emphasize the use of coordinated care management processes for patients. Those practices will retain the recognition for a three-year period, until another review to ensure they are meeting PCMH standards.
