The Regional Medical Center at Memphis and BlueCross have announced a partnership to develop patient-centered medical homes for BlueCross members in Memphis at the hospital's five Health Loop primary care sites, according to a news release by the hospital.
In the Health Loop medical homes, primary care physicians will lead professional teams of healthcare professionals responsible for the ongoing care of patients as well as encourage them to more actively mange chronic medical conditions and overall health. Personal health coaching will also be provided, according to the release.
Through the partnership, BlueCross will provide additional funding for health information technology, as well as an on-site care coordinator who engages in proactive outreach to patients and community.
The Med, as the hospital is often referred to, will seek accreditation from the National Committee on Quality Assurance, an accreditingorganization that established key standards for patient-centered medical home development, for all five Health Loop sites.
