Walgreens announced July 22 that its Walgreens Find Care platform will now include solutions for chronic care management.
Through connected devices and digital therapeutic solutions, patients with diabetes, asthma and chronic obstructive pulmonary disease can use the digital health platform.
Specifically, diabetes patients will have access to the Dexcom G6 Continuous Glucose Monitoring System, which is designed to give patients the ability to continuously measure their glucose levels. Asthma and COPD patients can access different solutions to better manage their conditions.
Walgreens Find Care is offering patients with asthma access to Propeller, which works by attaching a sensor to patients' inhalers. The sensor tracks medication usage and location data, and the provides patients the feedback through a mobile app.
Since launching in July 2018, Walgreens has partnered with 26 organizations to offer its digital health platform.
