Health services company Harvard Pilgrim Health Care identified a software issue with its enrollment data management system that led to some mailings being sent to the wrong address.
Four details:
1. The software issue changed some individuals' mailing addresses to a different address associated with the individual's health plan, according to a Harvard Pilgrim statement.
2. The issue affected around 1 percent of individuals insured by Harvard Pilgrim and was connected to mailings as far back as 2013. All of the individuals affected were dependents on another person's health plan.
3. Information included in the incident included names, telephone numbers, birth dates, service dates, provider names and treatment information. There were 8,022 individuals affected.
4. Harvard Pilgrim said it discovered the issue Sept. 21, corrected the affected addresses and enhanced the system update process.
