At least two employees of CHI Health in Omaha, Neb., have reported identity crimes after someone accessed the health system's payroll information with authorization, according to an Omaha World-Herald report.
The third party company that processes payroll and tax information for CHI was hacked in late February or early March, according to the report. The breach was discovered March 13.
Fewer than 100 employees are affected by the incident, which compromised income tax forms and Social Security numbers, according to the report.
Two women have reported suspected tax fraud, saying someone filed tax returns in their names, according to the report.
