Allscripts company CarePort Health, which provides post-acute outcomes management software to ACOs and hospitals, is teaming up with Georgia Hospital Health Services to provide care coordination solutions to hospitals statewide.
CarePort's platform will help GHHS, a subsidiary of the Georgia Hospital Association, provide real-time data on transitions of care for hospitals across the state. Under the new partnership, any Georgia-based healthcare provider can use CarePort's platform to view a patient's information and clinical history as the individual moves across the continuum of care.
"We understand the importance of transparency throughout care transitions and are committed to providing real-time patient data to improve patient outcomes," said Bill Wylie, GHA senior vice president of business operations, said in a news release. "Our partnership with CarePort is our latest effort to equip providers with the technology they need to provide highly coordinated, quality care."
GHA serves as an advocate organization for more than 170 hospitals in Georgia, and GHHS focuses on developing and providing programs for its member hospitals.
