Ardent Health Services expands AI care coordination to 98 locations

Nashville, Tenn.-based Ardent Health has expanded artificial intelligence-powered care coordination to 98 locations across its health system.

Ardent's partnership with CareHarmony, a health tech company focused on chronic disease management, has grown to four markets after its initial successful launch at Ardent's UT Health East Texas market based in Tyler. Primary care engagement there increased tenfold in the first three months.

"This technology allows us to proactively identify patients who may need additional help outside our clinics and provide them with support navigating social determinants of health by improving access to healthy food, transportation and affordable medication — issues that can really impact quality of life," Ardent President CEO Marty Bonick said in an October news release.

After the AI identifies patients in need of navigation, care coordinators call them monthly to help schedule appointments and tests, educate them on medications, and connect them with home health or other community resources. Patients also get access to a 24/7 help line. Ardent is partnering with CareHarmony for its push into value-based care and ACOs.

The program has expanded to Ardent subsidiaries Lovelace Health System in Albuquerque, N.M., Portneuf Medical Center in Pocatello, Idaho, and University of Kansas Health System St. Francis Campus in Topeka.
