Downy, Calif.-based SuperCare Health, a post-acute, in-home respiratory care provider, suffered a data security incident that exposed 318,379 patients' information, according to a March 25 breach notification.
On July 27, SuperCare Health discovered unauthorized activity on its systems.
An investigation found that an unauthorized individual had access to parts of its network from July 23 to July 27, potentially viewing files containing patient information such as names, addresses, dates of birth, hospital or medical group, patient account numbers, medical record numbers, health insurance information, testing, diagnostic, and treatment information, and claim information.
SuperCare Health said it found no indication that patient information has been misused.
Since the incident, the provider said it has conducted a review of its security safeguards and implemented additional security measures to better protect the personal and protected health information of its patients.
