Northwell Health looks to tackle social determinants of health

New York City-based Northwell Health has partnered with a software platform that aims to assist patients and providers overcome social determinants of health.

NowPow, a Chicago-based company, uses diagnostic codes in health records to identify patients' most urgent, nonmedical needs, such as mold remediation or lack of access to healthy foods.

Northwell will provide the services to around 1,200 Medicaid patients who are part of the health system's home healthcare program. The goal is to use matching logic and evidence-based algorithms to recommend community organizations that may help patients.

Additionally, the platform considers patients' geographic location, eligibility for services, languages spoken and other factors. Northwell physicians can then track patients. The system allows providers to share, track and coordinate referrals for patients.
