Louisville, Ky.-based Humana and Knoxville, Tenn.-based Summit Medical Group announced a seven-year agreement to create an accountable care organization in East Tennessee for Humana Medicare Advantage beneficiaries, effective Jan 1.
As an ACO, the arrangement refocuses care on improving quality outcomes and coordination while reducing costs. Summit Health patients with Humana Medicare Advantage insurance will benefit from services including transportation assistance, caregiver support, health management education and disease management services, according to the statement.
Summit Medical Group is a primary care organization with 53 practice facilities, four diagnostic centers, seven physical therapy centers, four express clinics, a central lab and sleep services center. More than 308,000 patients receive summit care from 215 physicians and more than 100 advanced practitioners in Tennessee.
