Miramont Family Medicine, a medical group practice based in Denver, and Humana have announced an accountable care agreement for Humana's Medicare Advantage HMO members.
The agreement will cover patients who receive treatment at any of Miramont's seven clinics in the Denver area. The agreement involves a pay-for-value system, with incentives tied to quality outcomes and care coordination. Through the partnership, providers and Humana will use population health management tools like predictive analytics, disease management and wellness programs.
"Miramont Family Medicine is very patient minded and focused on the goal of providing better care and achieving greater health outcomes for individuals at an affordable cost, all while ensuring the patient experience is enhanced," John Bender, MD, senior partner and CEO of Miramont, said in a news release. "This is a natural collaboration, given Humana's excellent programs, and we are pleased that Humana members can benefit from our clinicians' expertise."
