Humana and King's Daughters' Health in Madison, Ind., announced Wednesday a value-based agreement to improve health and better coordinate care for Medicare Advantage beneficiaries.
The new accountable care agreement aligns both partners' population health efforts, which include predictive analytics, chronic disease management and wellness programs. The agreement will tie value-based incentives to performance improvement and quality outcomes, as defined by the National Committee for Quality Assurance Healthcare Effectiveness Data and Information Set.
Humana now has more than 900 accountable care relationships in 43 states and Puerto Rico.
