Iora Health currently serves more than 9,000 patients at seven primary care practices. The four new practices, which are scheduled to start seeing Humana Medicare Advantage members on Oct. 1, will have care teams that include physicians, nurses, health coaches, medication management, behavioral health and social services resources.
