New Orleans-based Ochsner Health system developed a program using wireless weight scales that helped decrease its heart failure 30-day readmission rates by 40 percent. Here are three things to know about the program:
1. Ochsner Health sent more than 100 heart failure patients home with a Carematix Blipcare wireless scale, with daily weights sent to Ochsner's Epic EHR system.
2. Clinicians monitored patient weight and adjusted their medications accordingly.
3. The program resulted in higher patient engagement and reduced readmissions. Detailed information on the program will be presented during the 2014 American Heart Association Scientific Sessions Nov. 18.
