Transitions care planning and care coordination were among the missed opportunities to prevent 30-day readmissions at 18 California hospitals, according to a study in Medical Care.
Researchers studied readmissions at 18 Oakland, Calif.-based Kaiser Permanente Northern California hospitals. An average of 8.7 factors contributed to potentially preventable 30-day readmission. These factors were related to care during the index stay, the discharge process and follow-up care.
Opportunities to prevent readmissions include transitions care planning and care coordination; clinical care; logistics of follow-up care; advance care planning and end-of-life care; and medication management.
