Oregon has the most hospitals with an "A" safety grade from The Leapfrog Group, according to the organization's spring 2019 update.
Twice a year, Leapfrog assesses more than 2,600 acute care hospitals nationwide for patient safety performance using 28 quality measures compiled by the Leapfrog Group, CMS, the Agency for Healthcare Research and Quality, the CDC, and the American Hospital Association.
