The St. Louis VA Health System has remedied major deficiencies found by the Office of the Inspector General during a July inspection, according to a St. Louis Post-Dispatch report.
Inspectors discovered a number of quality deficiencies related to cleanliness, training and safety, including dirty exam rooms and linens, improper storage of fire extinguishers and oxygen tanks, insufficient documentation for informed consent in sedation cases, insufficient follow-up patient care and more.
St. Louis VA Health System officials agreed with the outlined findings and worked on a correction action plan. During a follow-up inspection, OIG investigators did not find any lingering problems from the July inspection.
