San Francisco Hospitals Reduce Readmissions by 11%, Save $32M

San Francisco-area hospitals participating in the Avoid Readmissions through Collaboration initiative have reduced readmissions by 11 percent since 2010, saving an estimated $32 million, according to a news release.

ARC supports participating hospitals in improving their transition of care processes—ensuring patients understand discharge instructions and facilitating communication between hospitals and post-hospital care providers.


ARC aims to reduce readmissions in participating hospitals by 30 percent by the end of 2013.
