The Transitional Care Survey was developed by Planetree, an organization devoted to advancing patient-centered solutions of care. The survey, which asks patients about items related to their ability to care for themselves outside of the hospital, is to be administered before discharge. In addition, Planetree has developed a worksheet for patients to understand the components of taking ownership over their care.
