The Agency for Healthcare Research and Quality offers educational resources to help providers improve quality of care.
The Practice Facilitation Handbook is a resource for supporting the training of new practice facilitators in primary care practices. It is an extension of AHRQ's Integrating Chronic Care and Business Strategies in the Safety Net, according to AHRQ.
The book contains 21 modules, each between 30 and 90 minutes in length, with material for learners and teachers divided into four parts: overview of facilitation, core competencies for facilitators, common tasks for facilitators and training for facilitators in patient-centered medical homes.
