Potential Measures for Clinical-Community Relationships is a supplement to the March 2013 Clinical-Community Relationships Measures Atlas, also from AHRQ. The measure set is intended to improve management of chronic care and preventative care.
The Potential Measures for Clinical-Community Relationships proposes 52 potential measures and a core set of 13 measures for assessing relationships between clinical and community institutions. The measures fill the gaps in measurement from its predecessor, the Measures Atlas.
