The American Hospital Association's Hospitals in Pursuit of Excellence: Accelerating Performance Improvement program has released a complimentary guide to implementing improvement initiatives in hospital settings.
The guide includes two new frameworks, the Improvement Continuum, which describes improvement activities in microsystems, care coordination, defined population health and community health, and the Leadership Action Model, which is a framework for how to appropriately implement the Improvement Continuum.
