The TMF Health Quality Institute, the Medicare Quality Improvement Organization for Texas, offers free clinical teach-back cards to help healthcare providers explain medical information to patients.
The clinical teach-back cards, developed in 2011, explain the concept of teach-back, which is to ask patients open-ended questions to ensure they understood instructions. The cards also include information on several drugs, such as angiotensin-converting enzyme inhibitors and angiotensin II receptor blockers, as well as quick nurse reference guides on national quality indicators for conditions such as heart failure and acute myocardial infarction.
Download the teach-back cards for use in your own facility.
