The Institute for Healthcare Improvement offers four free hospital transition how-to guides to improve care transitions as part of its State Action on Avoidable Rehospitalizations initiative.
IHI recently updated the four how-to guides on improving transitions to reduce avoidable readmissions, which include tested interventions. Each guide addresses a transition from the hospital to a different setting:
1. Community setting.
2. Skilled nursing facility.
3. Home healthcare.
4. Clinical office practice.
