Nineteen percent of children with at least one complex chronic condition experience at least one readmission within 30 days of discharge, according to a study in Hospital Pediatrics.
Researchers found readmission was linked to the number of discharge medications. Patients receiving at least eight medications upon discharge had a 30-day readmission rate of 29 percent.
Researchers concluded the number of discharge medications were a significant risk factor for readmission.
