Methodist Le Bonheur's Navigators Helped Drive Down Mortality, Readmissions

Methodist Le Bonheur Healthcare, based in Memphis, Tenn., found "navigators" who assisted with patients' transitional care helped reduce hospital rates of mortality and readmissions, according to a Gant Daily report.

Methodist's program elects hospital staff members to serve as navigators, who in turn work with volunteer liaisons of churches participating in Methodist's Congregational Health Network. The navigator and the church's liaison partner to ensure a smooth transition for church members once they are discharged, according to the report.

 



The health system compared outcomes for patients in the navigators program to patients not in program. Methodist found mortality rates and readmission rates were 50 percent and 20 percent lower for patients in the navigators program than those not in the program.
