Cincinnati-based Mercy Health announced that its nurse care coordinators have created the first national organization to support nurse care coordinators and population health management team members.
The Registered Nurse Ambulatory Care Coordinator Association will support care coordination and population health management by providing guidance and education. Later this year, RNACCa will offer a certification program in population health management through a partnership with Xavier University in Cincinnati.
RNACCa members will consist of nurse care coordinators and team members that support care coordination, such as social workers, patient-centered medical home coaches, case managers, registered dieticians, certified diabetic educators and pharmacists, according to the release.
