Johns Hopkins Researchers Examine Top Reasons Clinicians Avoid Reporting Errors

Based on 274 returned completed surveys, Johns Hopkins investigators found getting colleagues into trouble, liability and embarrassment in front of colleagues were physicians' and residents' top reasons for avoiding reporting of errors and near-misses.
