El Camino Hospital Cuts Readmissions 25% With Health IT

El Camino Hospital in Mountain View, Calif., reduced readmissions 25 percent by using data analytics and advanced telecommunications, according to a case study by the College of Healthcare Information Management Executives.

The hospital uses statistical analysis of its data to stratify admitted patients according to risk for readmission. The healthcare team then uses this information to intervene early when necessary and prevent a readmission.

El Camino Hospital also uses a technology called telepresence to initiate two-way videoconferencing with sub-acute providers. When patients transition from the hospital to a long-term care facility, staff from the hospital and long-term care facility communicate regularly about the patient, which enhances coordination of care and prevents readmissions.
