The Center for Improving Value in Health Care in Denver announced a new initiative that aims to eliminate 8,700 hospital readmissions, save $80 million in healthcare costs and help patients avoid 34,000 days in the hospital.
Healthy Transitions Colorado is a two-year initiative that will focus on coordinating care among providers during transitions to prevent avoidable readmissions. The initiative is managed by CIVHC and four operating partners: the Colorado Hospital Association, Colorado Rural Health Center, Colorado Health Care Association and CFMC, the Medicare Quality Improvement Organization for Colorado.
