The Agency for Healthcare Research and Quality's On-Time Pressure Ulcer Prevention module shows significant promise, reducing pressure ulcers by 57 percent across 12 nursing homes in New York.
The On-Time module uses standardized data documentation and reports to track and identify ulcer risk factors in residents. The reports include tracking of existing pressure ulcers, stagnant or worsening pressure ulcers and a monthly data compilation on pressure ulcer reduction improvement.
Healthcare providers in the study used the reports to develop process improvement efforts, such as changing care plans and discussing patient behavior with social workers.
The monthly rate of pressure ulcer incidents in participating nursing homes fell from 4.6 per 100 patients to 2.0 per 100 patients using the On-Time module.
Researchers suggest the On-Time module can be beneficial in reducing the number of pressure ulcer incidences.
