Hospitals in Minnesota reported 316 adverse events from October 2014 to October 2015, up from 308 in the same period a year prior, according to a Minnesota Department of Health report. Of the 316 most recent mistakes, 93 led to serious injury and 16 resulted in death. Here are the three most reported events.
1. Pressure ulcers were at the top of the list with 104 reported. Sixty-five percent of these were related to the patient being on their back for prolonged periods.
2. Patient falls were the second most frequent with 67 reported. Patient falls and pressure ulcers combined accounted for 54 percent of all reported adverse events in 2015.
3. There were 29 wrong site surgeries/invasive procedures reported. The most common root cause behind these events was lack of procedural efficacy when ordering and verifying correct implants before insertion.
