The Leapfrog Group has released spring 2014 Hospital Safety Scores, giving grades of A, B, C, D or F to the nation's hospitals based on their ability to prevent errors, injuries and infections. In the updated scores, 804 hospitals received an A grade.
Overall, there has been a 6.3 percent average improvement in hospital performance since 2012, according to Leapfrog, and 48 of 50 states have seen improvement in their mean scores. However, 150 hospitals received a D, 22 received failing grades and four states — Alaska, Idaho, Nebraska and Wyoming, as well as the District of Columbia — have no hospitals with an A grade. Maryland hospitals were not scored due to lack of sufficient publicly available data.

The following five states had the highest percentage of A hospitals in the most recent Leapfrog scores:
1. Maine — 74 percent
2. Massachusetts — 70 percent
3. South Dakota — 67 percent
4. Illinois — 57 percent
5. Hawaii — 50 percent
