7 Key Elements of Safe Transitions of Care

Successful transitions of care for patients require greater collaboration among healthcare providers, according to a Joint Commission report.

The new Hot Topics in Health Care, "Transitions of Care: The Need for Collaboration Across Entire Care Continuum," is the second paper in a series of reports on transitions of care planned for a three-year period by The Joint Commission enterprise (The Joint Commission, Joint Commission Resources and Joint Commission Center for Transforming Healthcare).


The Joint Commission examined published research, conducted focus groups of healthcare leaders and front-line workers and led on-site visits to healthcare organizations to better understand the elements of safe care transitions. The Joint Commission identified seven foundations of safe transitions of care for all healthcare organizations:

•    Leadership support
•    Multidisciplinary collaboration
•    Early identification of patients/clients at risk
•    Transitional planning
•    Medication management
•    Patient and family action/engagement
•    Transfer of information
