The data is from CMS' Hospital Compare website and was collected between April 2014 and March 2015. For reference, the national average rate of discharge information provision is 86 percent, according to Hospital Compare.
The following hospitals had at least 94 percent of patients say they were given information about what to do during their recovery at home.
Note: Hospitals with fewer than 100 completed patient surveys were excluded from the data sorting process.
