Since spring 2012, the Leapfrog Group has assigned letter grades to hospitals twice a year based on their patient safety performance. Forty-two hospitals have earned an A in every update, including the fall 2018 update released Nov. 8.
These 42 straight-A hospitals have maintained the highest possible grade 14 consecutive times. Here is a list of the hospitals by state:
