Data comes from a Vree Health infographic, utilizing CMS data on readmissions rates for AMI for 2011 and 2012.

1. South Dakota — 17.5 percent readmission rate for CHF — 12.9 percent decrease

2. Arizona — 21.3 percent readmission rate — 10.8 percent decrease

3. Kansas — 20.1 percent readmission rate — 9.4 percent decrease

4. New Jersey — 24.5 percent readmission rate — 8.1 percent decrease

5. Hawaii — 18.5 percent readmission rate — 8.1 percent decrease

6. Montana — 15 percent readmission rate — 8.0 percent decrease

7. Mississippi — 23.8 percent readmission rate — 7.5 percent decrease

8. California — 22.5 percent readmission rate — 7.0 percent decrease

9. Texas — 21.9 percent readmission rate — 7.0 percent decrease

10. New York — 25.1 percent readmission rate — 6.9 percent decrease
