National care quality rates of long-term care, rehab, skilled nursing facilities

Medicare-certified skilled nursing facilities, inpatient rehabilitation facilities and long-term care hospitals are required to report quality measure data to CMS under the Improving Medicare Post-Acute Care Transformation Act of 2014.

CMS collects and reports the care quality measure performance data on a national, state and provider level. 

Healthcare-associated infection rates are measured as standardized infection ratios. A value less than 1.0 indicates fewer infections than expected and a value over 1.0 indicates more infections than expected. 

A higher reported change in mobility score indicates a greater improvement between admission and discharge.

Here are the national performance rates from 2023, as reported by CMS in December:

Long-term care hospitals

  • Catheter-associated urinary tract infection rate: 0.717

  • Central line-associated bloodstream infection rate: 0.715

  • Clostridium difficile infection rate: 0.313

  • Risk-adjusted change in mobility scores at discharge for patients admitted on a ventilator: 6

  • Percentage of patients who experienced one or more falls with major injury during their stay: 0.1%

  • Percentage of healthcare personnel who got a flu shot for the current season: 71.5%

Inpatient rehabilitation facilities

  • Catheter-associated urinary tract infection rate: 1.107

  • Clostridium difficile infection rate: 0.395

  • Risk-adjusted change in mobility scores at discharge for patients admitted on a ventilator: 58.7

  • Percentage of patients who experienced one or more falls with major injury during their stay: 0.2%

  • Percentage of healthcare personnel who got a flu shot for the current season: 79.1%

Skilled nursing facilities

  • National observed healthcare-associated infection rate: 7.05

    • Number of facilities with HAI rates better than the national rate: 93
    • Number of facilities with HAI rates no different from the national rate: 10,990
    • Number of facilities with HAI rates worse than the national rate: 298

  • Percentage of residents who experienced one or more falls with major injury during their stay: 0.8%

  • Percentage of healthcare personnel who got a flu shot for the current season: 45%
