Long-term care facilities with highest, lowest rate of successful return to home

The U.S. national rate of successful observed discharges to a patient's community or home from a long-term care facility between Oct. 1, 2021, and Sept. 9, 2023, was 18.05, according to CMS data published Sept. 25. 

Long-term care facilities that participate in Medicare are required to report quality-care measures to CMS, which is then published quarterly on the CMS CareCompare website. 

Here are the long-term care facilities with the lowest and highest return to home rates between Oct. 1, 2021, and Sept. 9, 2023:
