As part of its bankruptcy restructuring, Brockway, Pa.-based Guardian Health has sold eight of the system's skilled nursing facilities to GBK Eight.
Guardian filed for Chapter 11 in the U.S. Bankruptcy Court for the Western District of Pennsylvania, according to an Oct. 30 news release from the health system.
The eight facilities are:
- Beaver Valley Healthcare and Rehabilitation Center (Beaver Falls, Pa.)
- Belair Healthcare and Rehabilitation Center (Lower Burrell, Pa.)
- Eldercrest Healthcare and Rehabilitation Center (Munhall, Pa.)
- Fairmont (W.Va.) Healthcare and Rehabilitation Center
- Havencrest Healthcare and Rehabilitation (Monongahela, Pa.)
- Highland View Healthcare and Rehabilitation Center (Brockway, Pa.)
- Meadowcrest Healthcare and Rehabilitation Center (Bethel Park, Pa.)
- Oak Hill Healthcare and Rehabilitation Center (Greensburg, Pa.)
Guardian also transferred operations of 11 skilled nursing facilities to Toms River, N.J.-based Oxford Valley Health:
- Clarion (Pa.) Healthcare and Rehab Center and Clarion Personal Care
- Haida Healthcare and Rehab Center (Hastings, Pa.)
- Kinzua Healthcare and Rehab Center (Warren, Pa.)
- Oil City (Pa.) Healthcare and Rehab Center and Oil City Personal Care
- Richland Healthcare and Rehab Center (Johnstown, Pa.)
- Shippenville (Pa.) Healthcare and Rehab Center
- Titusville (Pa.) Healthcare and Rehab Center
- Uniontown (Pa.) Healthcare and Rehab Center
- Walnut Creek Healthcare and Rehab Center (Erie, Pa.)
- Waynesburg (Pa.) Healthcare and Rehab Center
- William Penn Healthcare and Rehab Center (Lewistown, Pa.)
