Downers Grove, Ill.-based DuPage Medical Group and health insurer Humana have partnered to open an outpatient clinic for Chicagoland senior citizens who have chronic diseases.
The clinic, called BreakThrough Care Center, opened Jan. 2 and has two locations.
The clinics will focus on seniors who have complex, chronic conditions with the goal to reduce hospitalizations and decrease medical costs. Each clinic includes a registered nurse, health coaches, nutritionists, medication therapy management, physical therapists and social services resources.
Most patients will be Humana Medicare Advantage plan members with chronic or complex conditions, though other patients might be accepted based on referrals, according to the release.
