Arizona Care Network, formed through collaboration between San Francisco-based Dignity Health and Phoenix-bassed Abrazo Health, and UnitedHealthcare have announced they are forming an accountable care organization.
The new ACO is set to launch May 1. Under the agreement, Arizona Care Network and its affiliated physicians will provide coordinated care to nearly 15,000 Phoenix-area members of UnitedHealthcare's employer-sponsored health plans.
