Watertown, Mass.-based Tufts Health Plan and Cambridge, Mass.-based Iora Health announced Wednesday a new primary care collaboration for the health plan's Medicare Preferred HMO and Senior Care Options members.
As part of the collaboration, Iora will open two new primary care practices in the Boston area that deliver care under a team-based model in which a physician, health coach and behavioral health specialist work together to coordinate patient care.
Patients will benefit from same-day or next-day visits and 24/7 access to a provider for urgent care services. Providers will also offer email, phone or video-chat connectivity.
The two new locations are set to open in September.
