Camp Hill, Pa.-based RiteAid announced Reliance ACO, a Farmington Hills, Mich.-based accountable care organization, will become the eighth partner part of the Rite Aid Health Alliance initiative for coordinated care.
Through the initiative, physicians, Rite Aid pharmacists and care coordinators work together to provide comprehensive care for patients with chronic conditions. The program is unique in that physicians can refer patients to pharmacists and in-store care coaches for additional coaching on medication management and meal and exercise planning.
Patients are referred into the program by their primary care physician, and then have access to medication compliance support, comprehensive medication reviews and reconciliation, nutrition and weight management information, disease education, exercise coaching and tobacco cessation support. All interactions will be recorded electronically in patient profiles to support continuous care.
