Community Health Network Partners With Walgreens Take Care Health Systems

Indianapolis-based Community Health Network, an eight-hospital system, and Walgreens subsidiary Take Care Health Systems have announced a clinical collaboration agreement in which the walk-in clinics will act as extensions of Community Health Network.

The agreement aims to facilitate more coordinated health services and improve patient access to high-quality care through Walgreens Take Care Clinic locations in Indianapolis, Lawrence and Carmel, Ind. Components of the agreement include:

•    Care coordination for Community Health Network patients with Take Care Clinic providers
•    Direct communication between Community Health Network physicians and Take Care Clinic nurse practitioners to help coordinate care and share patient information
•    Sharing Community Health Network service information with Take Care Clinic patients and informing Community patients about Take Care Clinic services

The two organizations are also discussing ways to share electronic medical records, with the end goal being full EMR integration.

"Through this relationship, we are bringing new thinking to local healthcare delivery and helping ensure that patients are receiving the best care at the best location," Tim Hobbs, MD, chief physician executive at Community Health Network, said in a release. "This type of collaborative approach leverages the unique capabilities of both parties and represents the direction that healthcare is moving."
