The following is a list of hospitals with the lowest 30-day mortality rates from heart failure, according to data from CMS.

Data was pulled from Hospital Compare and is from July 2010 through June 2013, the most recent data available.

All the hospitals on this list have a 30-day mortality rate from heart failure of 8.6 percent or less. For reference, the national average is 11.6 percent.
