A missing controlled substance monthly report prompted a data breach notification from the Veterans Affairs Medical Center in Washington, D.C.
The report contained 1,062 veterans' names and full and partial digits of their Social Security numbers, and was discovered missing March 31.
The medical center notified affected veterans and says it is taking the appropriate steps to protect their identities.
